Case Management Officer (GOVT HOSPITAL) *NURSE* #HZT

• Administer brief psychogeriatric assessments (MMSE, ADL, iADL) and brief child assessment (Continuous Performance Testing) and collect basic socio-demographic information for all new outpatient patients and follow-up patients as required.
• Obtain clinical information from patients via use of scales (ADHD, depression, anxiety rating scales) prior to child/adolescent first visits in the clinic.
• Contact patients prior to their first visits to confirm their attendances and remind them to attend their first visit and follow up appointments on time.
• Triage cases based on their clinical needs under the supervision of a Psychiatrist.
• Conduct home/ school visits when necessary.
• Competently perform routine and complex neuropsychological assessments for patients with cognitive complaints, under the supervision of a Senior Psychologist.
• Provide psycho-education for patients and their families/caregivers.
• Assess caregivers’ needs with appropriate assessment tools and arrange appropriate referrals for support.
• Collaborate with Allied Health staff such as Medical Social Workers, Psychologists, Occupational Therapists and Pharmacists for the management of patient’s assessed needs.
• Monitor patient’s appointments, investigations and social and rehabilitation plans.
• Facilitate discharge to step-down care (polyclinics, GP clinics, SLH etc) once diagnosis and care plans are finalized.

Secondary Responsibilities and Duties:

• Ensure that relevant patient education materials are available and are up to date; these may be sourced from AIC, HPB, VWOs, etc.
• Review key performance indicators such as waiting time, default rate, etc. timely to promote continuous process improvement.
• Cross cover for other patients who are in need of case management under the care of psychological medicine department.

Job Requirements:

Qualification:

• Degree in Nursing, Psychology or relevant discipline.

Experience:

• Relevant experience will be advantageous.
• Good knowledge of geriatric, adolescent/child care and community skills.
• Ability to communicate effectively with multi-racial groups of patients.
• Ability to multi-task and work independently and work as a team.
