Care Manager

As a Care Manager you will be responsible for the following duties:

• Identify and assist patients who require care management plans
• Monitor residents’ daily schedules and care plans
• Monitor and track patient's progression to ensure optimal clinical and non-clinical outcomes
• Engage with residents and family members to determine satisfaction with the quality of care
• Work with the clinical team and stakeholders to provide quality care to residents
• Document and ensure confidentiality of patient's records
• Participate in quality improvement projects
• Perform or participate in outreach programs or activities
• Any other duties required within the team

Requirements:

• At least a Diploma or Degree in Nursing with valid SNB registration
• Candidates with an Advanced Diploma in a specialty area will be a plus
