Care Coordinator (STAFF NURSES/3 months) - up to $4k/West #HAO

Salary: up to $4000

Duration: 3 months

Working Days & Hours: Mon-Fri Office Hours

Location: Bukit Batok

Job Description:

• Participate in discharge planning and identify patient requiring post discharge care transition support.

• Plan and carry out post discharge support and follow up via phone call and/ or home visit to ensure patients and caregivers’ ability to cope at home and discharge care plan is implemented.

• Assist patient and their families to streamline and coordinate the range of community service available.

• Implement timely and appropriate care coordination and evaluate the outcome accordingly.

• Act as liaison person, collaborating with patients, families and the multidisciplinary teams to achieve best outcome for the patients

• Monitor patient’s general medical condition during home visit and report to clinician if necessary

• Promote positive changes in patient’s lifestyle in the community.

• To help patients and their families to plan for and improve end of life care

• To participate in hospital activities that contribute towards improving the quality of patient care such as research, quality or innovative projects that enhance patient care

• Support data collection and preparation of reports and statistics for outcome tracking.

Job Requirements: Min Degree in Nursing w min 3 yrs of relevant nursing experience
