Overview and Purpose of the Position
We are seeking a dedicated and compassionate Care Coordinator to join our Breast Cancer Programme team. The Care Coordinator will apply advanced knowledge of the health needs, preferences and circumstances of patients with breast cancer to navigate the patient and family through the health system and optimize individual health and well‑being at all phases across the continuum of cancer care, including diagnosis, treatment, rehabilitation, follow‑up, survivorship and palliative care. The Care Coordinator will maintain close communication with patients across the care continuum to ensure coordination, efficient delivery, and removal of care barriers. They will be an integral member of the multi‑disciplinary Breast Oncology team at NCIS at NTFGH/NUH, providing coordinated and systematic clinical care to comprehensively address the needs of breast cancer patients.
Job Responsibilities
- Provide individualized assistance, both informational and emotional support, to patients and their families to overcome healthcare system barriers and improve continuity of care.
- Identify and navigate high‑risk patients through the care journey to ensure they do not fall through gaps and have access to necessary resources.
- Follow up with patients via phone as needed to improve access to the medical team and reduce unnecessary emergency department visits and admissions.
- Assist patients and families in transitioning across different services and locations.
- Communicate effectively with healthcare professionals to facilitate smooth transitions.
- Assess patients’ physical, psychosocial, and spiritual needs; identify barriers to care; initiate interventions to address identified needs.
- Evaluate patient navigation outcomes.
- Ensure compliance with care pathways and streamline care within NUHS, primarily between NUH and NTFGH.
- Ensure smooth transition of care and appropriate right‑siting of patients based on care pathways.
- Ensure timely and appropriate referrals to specialists, allied health, and support services.
- Coordinate care between services (medical, surgical, allied health) and settings (inpatient, outpatient, community).
- Maintain close liaison with Medical Oncologists, Surgical Oncologists, Radiation Oncologists, Radiologists, and other specialists involved in the patient’s care.
- Act as tumour board coordinator for the weekly tumour board, ensuring the database is complete and up‑to‑date; manage and audit the tumour board database; cross‑cover other tumour boards to maintain continuity during colleague leave.
- Provide patient and family education pertaining to the cancer journey; triage patients to subspecialty resources (e.g., stoma nurse, oncology APN, MSW, financial counsellor).
- Advise and guide patients on estimated treatment costs and financial counseling; develop and update patient education materials in collaboration with the multidisciplinary team.
- Support multidisciplinary team members on patient care matters; advise clinic staff on cancer treatments, procedures, and financial counselling.
- Obtain key clinical and operational data at transitional checkpoints; enter data into the cancer database in a timely manner; compile data for audits and simple analytics.
- Participate in quality improvement projects aimed at improving coordination of care and optimizing outcomes for patients and services.
- Liaise with research assistants, residents, and research fellows on projects related to breast cancer.
- Undertake other responsibilities as assigned by the reporting officer or department head; provide flexible support for cross‑team initiatives and evolving institutional priorities.
Qualifications
- Good Class Honours Bachelor’s degree, preferably in Nursing or a related allied health field.
- Excellent interpersonal and communication skills; fluent in spoken and written English; additional languages highly regarded.
- Mature and pleasant personality with a can‑do attitude, drive, and initiative.
- Ability to work independently and well under pressure; collaborative and team‑orientated.
- Efficient time management; able to prioritize, multi‑task, and be flexible.
- Computer proficiency essential – M365 (Word, Excel, PowerPoint, Teams, Outlook).
- Proficiency in database management and use of database tools (e.g., Redcap, EPIC EMR system) highly regarded.
