A leading company in Singapore seeks a Care Coordinator to support the clinical team in various administrative and case management tasks. The role involves advising clients on financial schemes, managing client onboarding, and ensuring a seamless client experience. Ideal candidates should possess at least a Diploma, preferably in healthcare, and exhibit strong interpersonal and communication skills.
The Coordinator is responsible for providing support to the clinical team in the assessment, case work, programmes and supporting the clients and their families to resolve personal, social, emotional, and financial issues.
He/ She will provide service coordination management, administrative support, and operations improvement. Care Coordinator will need to work collaboratively with colleagues from other departments to ensure a seamless client experience for our service users.
