Care Coordinator

Job Description

• Assist to engage the residents in interactive activities, exercises, money management activities, purposeful occupational tasks, and community mobility activities.
• Coordinate and carry out any other tasks assigned by the Physiotherapist and the Occupational Therapist when necessary.
• Assist the multi-disciplinary team to plan, roll out activities and track progress of the residents and day care clients.

Requirements

• Mid-Career change candidates are welcome to apply.
• Previous work and/or volunteer experience in the care industry (e.g. childcare, rehabilitation, senior care centre, nursing home, palliative care facility) will be an added advantage.
