Book or reschedule appointments based on caller's requests.
Handle any other hotline administrative functions such as handling of emails, referral requests and emergency activations.
Engagement of residents and community partners to address enquiries about population health and services and initiatives and link up with the appropriate team/service where appropriate.
Support the navigation and provision of community-based resources/services to support residents and caregivers to take charge of their own health.
Conduct outbound nudges to encourage enrolment and participation in screening and vaccination.
Proactive team player to contribute to service improvement initiatives and projects and be part of implementation.
What We Are Looking For
Recognised Diploma in any disciplines.
Minimum 1 year of related experience in healthcare or service industry.
Proficient in MS Office, good planning and organization skills.
Possess strong communication and interpersonal skills.
Willing to work on rotating shifts, including PHs and weekends.
