Job description

• Conduct comprehensive assessments to understand the patient's and family's social, emotional, psychological, and financial needs.
• Provide counseling and therapeutic interventions to help patients and families cope with illness, grief, loss, anxiety, and other emotional challenges.
• Offer crisis intervention and support during medical emergencies or challenging situations (e.g., domestic violence, self-harm, neglect, abuse).
• Address issues related to illness coping, illness anxiety, grief and loss, and bereavement.
• Collaborate with the medical team to assess patients' care needs and facilitate safe and timely discharge plans.
• Coordinate and arrange for post-discharge care, including home care services, rehabilitation programs, nursing home placements, and assistive equipment.
• Provide information and support to caregivers, helping them navigate healthcare services and access community resources.

Requirement

• 2 - 3 Years of working experience
• Keen in the medical field
