Responsibilities:

  • Provide case management and appropriate level of care through risk assessment, care planning, and timely interventions.
  • Collaborate with schools and community stakeholders to implement holistic intervention and care plans.
  • Conduct and facilitate psycho-educational group sessions for youth and families.
  • Assess cases using structured tools and implement interventions effectively.
  • Maintain accurate and up-to-date case documentation and reports.
  • Support other duties and special projects as assigned by supervisor or management.

Requirements:

  • Minimum Degree or Postgraduate qualification in Social Work, Counselling, or Psychology.
  • At least 4 years of relevant experience working with at-risk youth.
  • Proficient in casework and group work.
  • Strong report writing and communication skills.
  • Able to engage and build rapport with youths effectively.
  • A good team player who can also work independently with minimal supervision.
  • Trained in YLS/CMI (Youth Level of Service/Case Management Inventory) is required.
