Duties and Responsibilities
1) Program Coordinator – Transitional Shared Care Programme (TSCP) for Cardiology Patients
- Oversee daily operations of the TSCP for cardiology patients, which includes:
a. Screening patients in cardiology clinics for eligibility based on specified criteria for each condition group and enrolling suitable patients into the program.
b. Consulting with the attending physician to confirm patient suitability.
c. Coordinating with care coordinators from primary care providers (such as NUP, Primary Care Networks, and General Practitioners) to schedule follow-up appointments.
d. Sharing patient treatment targets with primary care partners.
e. Managing and arranging follow-up appointments at cardiology clinics.
f. Following up with patients who miss appointments.
g. Providing health education and counselling to patients. - Monitor and track the performance of the TSCP:
a. Develop and maintain a database to record all enrolled patients.
b. Document attendance records for both primary care and cardiology clinic visits.
c. Track patient progress and ensure adherence to clinical protocols.
d. Monitor and record the achievement of treatment targets.
2) Clinical Audit Coordinator
- Collect and submit data on acute myocardial infarction mortality to the Ministry of Health (MOH) within stipulated timelines.
- Prepare reports for the Head of Department and submit them to the Clinical Governance Office under Medical Affairs.
- Gather clinical data, including diagnostic details, medications, procedures, comorbidities, lab results, and treatment plans (e.g., medical management or revascularization procedures).
- Ensure accurate and timely submission of audit data to relevant authorities.
- Support additional operational and administrative duties as assigned by the supervisor.
Key Requirements
- A Bachelor’s degree from a recognized institution.
- Clinical background such as Enrolled/Registered Nurse or Allied Health Professional is an added advantage.
- 3 to 5 years of experience in a healthcare setting.
- Strong verbal and written communication abilities.
- Excellent interpersonal skills with the ability to collaborate effectively across various levels of staff.
- Proactive and positive attitude.
Team player with the ability to work independently when needed
