Job description

Responsibilities

  • Build and maintain regular engagement with key community partners to improve care integration and community-based care delivery.

  • Manage feedback, concerns, and operational issues raised by or involving community care partners.

  • Stay informed on developments within the ILTC and community care landscape.

  • Identify and explore new opportunities for collaboration with community care organisations to augment services and support.

  • Lead or support the planning, implementation, and tracking of joint initiatives and projects.

  • Serve as secretariat for workgroups and inter-agency meetings as required.

  • Support the implementation of Population Health strategies and regional health plans.

Requirements
  • Degree, preferably in Life Sciences, Business Administration, or a related field.

  • Min 3 years relevant experience, ideally in community care, healthcare partnerships, or programme/project management.

  • Demonstrated ability to manage projects and work collaboratively with multiple stakeholders.

  • Strong interpersonal skills with the confidence to engage internal and external stakeholders at all levels.

  • Capable of multi-tasking and managing tight timelines.

  • Excellent organisational, analytical, and problem-solving skills.

  • Strong verbal and written communication, presentation, and negotiation skills.
