Assistant Manager, Intermediate and Long Term Care (Partnerships and Projects), RHSO

Reporting to Manager, Intermediate and Long Term Care (ILTC), the Position Holder will be responsible for engagement of ILTC providers (e.g., nursing homes, day care centers, home care services) and management of collaborations between NUHS and ILTC providers in the community to enhance care integration and delivery of care in the community.

Job Responsibilities

  • Regular engagement with key account community partners to improve care integration and delivery of care in the community.
  • Management of feedback and issues with/from community care partners.
  • Keeping abreast of developments within ILTC and community care organisations.
  • Identifying opportunities for collaborations with key partners to improve care integration and augment community care.
  • Management of collaborations/initiatives/projects to improve care integrations between NUHS and ILTC providers and/or augment community care.
  • Serving as workgroup/meeting secretariat.
  • Supporting implementation of NUHS Population Health strategies and regional health plan.

Job Requirements

  • Good Class Degree, preferably in Life Sciences, Business Administration or related qualification.
  • 3 to 5 years of relevant working experience; prior experience in community care or partnerships will be an advantage.
  • Experienced in project management and working with multiple stakeholders.
  • Possess confidence to engage internal and external stakeholders at all levels.
  • Ability to multi-task and work under tight timelines.
  • Good analytical and organizational skills.
  • Excellent written and verbal communication, problem solving, presentation, negotiation, and interpersonal skills.
