Job description

You will be part of the Healthier SG team that comprises Primary Care Providers, Community Nurses, Wellbeing Coordinators, and Community Partners to provide efficient and effective integrated person-centred health and social care to residents residing in the SingHealth region.

Responsibilities:

  • Contribute to this care team by directly overseeing the Wellbeing Coordinator members and/or monitoring and managing their performance.
  • Serve as a touchpoint to SingHealth residents and patients, and journey with them towards better health.
  • Provide strong support in managing manpower utilization, staff coverage, and staff deployment in each community of care to ensure smooth operations.
  • Identify and address the issues and concerns faced by each community of care, and provide solutions to support teams in delivering effective and efficient service to the clientele group.
  • Ensure each community of care maintains regular and ongoing contact with their clients through regular phone reviews and home visitations as required.
  • Keep team members up to date on new services in the community and motivate them to provide effective information and referral services to their clients and caregivers, matching the latter to the right service providers.
  • Establish and maintain strong community networks with other agencies within the designated community of care.
  • Engage community partners and stakeholders to proactively identify residents with potential issues.
  • Work with community partners to promote Healthier SG.

Requirements:

  • Degree from recognized institutions, preferably in Social Work or a healthcare-related discipline or equivalent.
  • Preferably a Registered Social Worker (RSW) with the Singapore Association of Social Workers, or with the Case Management Society of Singapore (CMSS).
  • At least 6-8 years of experience and demonstrated competency with clinical experience in the Social Work field in medical and/or community settings.
  • At least 2 years of experience in a supervisory role.
  • Ability to speak multiple languages and dialects would be advantageous.
  • Able to work with a flexible schedule, such as weekends and evening hours, due to community events.
