Reporting to the Director of Operations at RHS Office, the incumbent will lead efforts in developing healthy precincts and building community partnerships to promote community health.
Healthy Precinct refers to a geographical area with an environment and culture that support residents in achieving and maintaining healthy lifestyles.
Job Responsibilities:
Lead the development of the overall strategy and framework for Healthy Precincts.
Plan and implement Health Precincts to achieve health-social integration with internal and community stakeholders.
Develop and monitor work plans and workstreams related to Healthy Precincts.
Create and execute plans to empower the community to take charge of their health.
Foster a community culture that promotes healthy behaviors and emphasizes preventive care with stakeholders.
Build partnerships with internal and external stakeholders such as grassroots organizations, social service agencies, and healthcare providers to maintain patient and resident health and ensure integrated care across settings.
Develop and implement initiatives aimed at integrated care and population health improvement.
Lead and support the team in achieving these goals.
Ensure the department is adequately resourced in terms of personnel, funding, skills, and technology.
Develop team members' skills and potential.
Promote integration with other RHSO departments and divisions.
Requirements:
Bachelor's Degree in healthcare, business, or related fields.
At least 10 years of relevant experience in healthcare, corporate/strategic planning, and partnership development, including a minimum of 5 years in leadership roles.
Proven ability in strategic thinking and planning.
Excellent writing skills.
Strong interpersonal and communication skills.
