We are seeking a motivated and dedicated Antimicrobial Stewardship Pharmacist to join our dynamic team. As a key member of our healthcare team, you will collaborate with diverse clinical teams to optimise patient care, champion appropriate antimicrobial use, combat antimicrobial resistance, and drive innovative stewardship initiatives.

Key Responsibilities includes:
· Conduct regular audits of antimicrobial prescribing practices and provide timely, constructive feedback to prescribers to promote appropriate antimicrobial prescribing
· Perform antimicrobial therapeutic drug monitoring (e.g., beta-lactams) and provide individualized dosing recommendations based on patient-specific factors, pharmacokinetic and pharmacodynamic properties of the antimicrobial
· Develop and implement targeted stewardship interventions to improve antimicrobial prescribing practices
· Provide expert advice on appropriate antimicrobial therapy to healthcare professionals
· Contribute to research projects related to antimicrobial use and antimicrobial resistance
Requirement:
· Eligible for registration with the Singapore Pharmacy Council
3 years of working experience in hospital pharmacy practice (with prior experience in antimicrobial stewardship work or board certified in pharmacotherapy is preferred)
