Ambulatory Care Clinical Pharmacist (Manage Chronic Disease Only!) Up 6K #HJC

- Full registered with SPC
- Mon Tue 830am-6pm and Wed to Fri 850am-545pm. 1 in 4 Saturdays (4 hrs)

Key responsibilities
  • Provide clinical pharmacy services in outpatient clinics, with focus on chronic disease management (e.g. diabetes, hypertension and anticoagulation) and medication optimisation
  • Collaborate with multidisciplinary teams to enhance therapeutic outcomes and are transitions
  • Participate in service development, quality improvement initiatives and patient education
Requirements
  • Full registration with Singapore Pharmacy Council
  • Bachelor Degree / Masters in Pharmacy
  • Good written and communication skill
  • Strong foundation in chronic disease pharmacotherapy (e.g., diabetes, hypertension, anticoagulation)
  • Skilled in therapeutic monitoring, medication reconciliation, and deprescribing
  • Familiarity with evidence-based guidelines and ambulatory care protocols
  • Engages in teaching, research, or professional advocacy
  • Experience in collaborative care model willbeanadvantage
