Assesses, plans, implements, evaluates, and documents nursing care of patients in accordance with organizational policies and standards of professional nursing practice, utilizing the framework for professional nursing practice and development.
Ensures quality of care by adhering to therapeutic standards; measuring health outcomes against patient care goals and standards; making or recommending necessary adjustments; and following hospital and nursing division's philosophies and standards.
Provides comprehensive, safe, and effective evidence-based nursing care to achieve identified health outcomes. Promotes a hearing environment.
Prepares rooms, sterile instruments, equipment, and supplies, and ensures that stock of supplies is maintained.
Demonstrates high-level communication skills.
Refers and coordinates patients' discharge arrangements with the case manager in accordance with DSFH discharge policy.
Performs other QI responsibilities as delegated from time to time by the Nurse Manager.
Creates partnerships with patients and families to establish goals and plans for delivery of patient-centered care.
Performs duties as per the code of conduct requirements and ethical standards outlined in the HCM policies and guidelines.
