Job description

Job Description Summary

The Home Healthcare Nurse 3 provides advanced-level nursing care in the assigned nursing unit and is responsible for supporting operational processes. This includes, but is not limited to, maintaining high standards of patient care, assisting in resolving complex clinical issues, mentoring intermediate-level nurses, and collaborating closely with the multidisciplinary team to ensure efficient care delivery.

  1. Contribute to strategic initiatives aimed at improving the nursing practices within the assigned units.
  2. Participate in developing protocols and processes to enhance patient safety and operational efficiency.
  3. Pursue high staff engagement and obtain high patient experience scores.
  4. Achieve and sustain high quality of care and patient safety.
  5. Support the empowerment of nurses through the enculturation of the JHAH Nursing Professional Practice Model.
  6. Deliver high-quality nursing care services, focusing on complex cases.
  7. Assist in managing workflow, ensuring proper resource utilization, and mentoring junior nurses.
  8. Ensure adherence to clinical protocols and participate in evaluating patient outcomes.
  9. Meet the mandated activities for level 3, achieving a minimum of 20 points across at least four categories within each 12-month cycle as outlined in the Clinical Advancement Program.
  10. Utilize the nursing process to assess, plan, implement, evaluate, and document patient care from admission through discharge, specific to the patient's age.
  11. Provide nursing care and patient-family education aligned with the plan of care, respecting privacy, dignity, personal preferences, and cultural considerations, demonstrating acceptance, caring, and empathy, and utilizing appropriate resources when applicable.
  12. Precept new hires and trainees during their probation/training period, and participate in making recommendations regarding their progress and training needs.
  13. Respond promptly and effectively to emergency and lifesaving situations by utilizing clinical skills, critical thinking, and evidence-based practices to ensure optimal patient outcomes.
  14. Perform a wide range of nursing procedures with attention to accuracy, patient safety, and comfort, maintaining a safe, clean, and organized environment for patients, families, visitors, and staff.
  15. Perform procedures requiring advanced competency skills or training in the expanded nursing roles of Home Healthcare.
  16. Present a positive image of JHAH through professional appearance and behavior during home visits.
  17. Immediately escalate abnormal findings to the Charge Nurse or Home Healthcare physician and advise patients/families to attend Emergency Services when further assessment is necessary.

Minimum Education Required: 3 years International Diploma or Bachelor Degree in Nursing

Professional Certifications Required: Current licensure as a registered nurse, Basic Life Support (BLS), Advanced Cardiac Life Support (ACLS)

Years of Experience: Minimum 4 years of nursing experience, including at least 2 years in Home Healthcare or a related field.
