Job description

Abstract clinical information from Medical Record and assigning appropriate ICD coding and reverse to DRG.

Roles and Responsibilities:

  • Assign codes to diagnoses and procedures, using ICD (International Classification of Diseases).
  • Account for coding and abstracting of patient encounters, including diagnostic and procedural information, significant reportable elements, and complications.
  • Follow coding conventions and serve as a coding consultant to care providers.
  • Comply with medical coding guidelines and policies.
  • Validate the codes and DRG assignment appropriateness to ensure consistency and efficiency for inpatient.
  • Review and verify documentation supports diagnoses, procedures, and treatment results.
  • Ensure the codes are applied correctly and sequenced appropriately.
  • Research, analyze, recommend, and facilitate a plan of action to correct discrepancies and prevent future coding errors.
  • Identify reportable elements, complications, and other procedures, serving as a resource and subject matter expert to other coding staff.
  • Communicate with other clinical staff regarding documentation.
  • Audit clinical documentation and coded data to validate documentation supports services rendered for reimbursement and reporting purposes.

Knowledge and Experience:

  • Experience in medical coding and quality for +2 years.

Education and Certifications:

  • Diploma in Medical record / scientific sciences fields.
  • Associate's Degree in Medical coding or successful completion of a certification program.
