Job description

POSITION OVERVIEW

As a Healthcare Educator, the ideal candidate assesses learning needs, develops an education plan, provides education, and evaluates the outcome of education of patients and caregivers with diabetes. The Healthcare Educator provides education in the inpatient and/or outpatient setting and participates on teams for policy and protocol development.

KEY RESPONSIBILITIES

  • Conduct group classes and one-on-one consultations to educate patients on diabetes management at basic, intermediate, advanced, and gestational levels.
  • Provide dietary counseling, including carbohydrate counting, label reading, meal planning, and understanding dietary fats.
  • Apply motivational interviewing and behavior modification techniques to encourage positive lifestyle changes and self-management skills.
  • Offer nutrition guidance on conditions such as hyperlipidemia, hypertension, weight management, and gastrointestinal disorders.
  • Monitor and document patient progress, preparing reports on diabetes management outcomes.
  • Assess and identify potential and diagnosed cases of diabetes.
  • Educate patients and families on diabetes care, treatment adherence, and lifestyle modifications.
  • Collaborate with multidisciplinary teams to enhance patient care and support networks.
  • Lead and facilitate diabetes support groups.
  • Advocate for diabetes awareness and prevention through internal and external initiatives.
  • Maintain professional licensure by complying with all continuing education and certification requirements.
  • Participate in committees, task forces, and team initiatives as needed.
  • Adhere to all policies, procedures, and set standards.
  • Engage in ongoing training, education, and professional development.
  • Perform additional duties as assigned.

IDEAL QUALIFICATIONS

  • Relevant Master's (Preferred) or Bachelor's Degree required.
  • Minimum of 2 years of experience.
