Job Responsibilities:
- Performs psychosocial assessment and counseling therapy to patients and their families.
- Provides follow-up services to appropriate patients and families.
- Participates in the preparation of the treatment plan, especially for patients with psychosocial issues, and their treatment requires resources such as family support, transport, and finance.
- Participates in preparing the discharge plan to help patients return to their home environment with optimal functioning.
- Responds to referrals for inpatients, outpatients (RDU, special needs, and kidney transplant patients), and ER, as per policy.
- Follows up on patients for whom initial assessments are done when indicated.
- Helps families cope with loss and grief situations (Code Mercy and Code Blue).
- Provides support to DNR (Do Not Resuscitate) patients’ families.
- Assists patients facing financial needs by conducting case studies and referring them to appropriate charity channels.
- Contacts patients who refuse admission or continuation of case to ensure they are in stable condition and understand reasons for DAMA, as per policy.
- Provides special care for patients with special needs, such as the terminally ill, handicapped, victims of abuse or domestic violence, and those with social problems not psychiatric in origin.
- Coordinates reporting of abuse and domestic violence cases to MOH and HRSD, and reports cases to the Violence and Abuse Protection Committee.
- Participates in the multidisciplinary discharge planning team.
- Coordinates periodic visits of charity institutions to inpatients.
- Issues Priority to Them Cards and parking stickers for Special Need patients as per policy.
- Helps communicate with clergy and interpreters if needed for patients.
- Coordinates with the accounting department to define financial covers and limits.
- Attends recommended committees and meetings.
- Performs other duties within the scope of responsibility and job requirements.
- Arranges family/physicians meetings, including preparation and attendance.
- Provides screening and support to patients and families in ICU, CCU, NICU, and PICU.
- Maintains an updated list of interpreters, clergy, and sign language services for patient communication.
- Maintains extensive information on community resources and programs, making this available to patients and families.
- In kidney transplant cases, conducts psychosocial evaluations, presents data, addresses problems to the transplant committee, and educates patients on post-transplant expectations, medications, and insurance coverage.
- Demonstrates compassionate communication skills, ensuring patient-centered care.
