Job description

Key Responsibilities and Duties:

  1. Acts as drug information and education resource for patients, nurses, and for medical clinical staff and for other healthcare professionals.
  2. Assists medical staff with the selection of drug therapy and their doses and with choosing parameters of patient monitoring and shares the responsibility for patient-specific pharmacotherapy plan to ensure optimal outcomes.
  3. Responsible for maintaining and periodically revising Fakeeh care medications master list and medications formulary list through strong involvement and permanent membership of the pharmacy and therapeutic committee (P&TC).
  4. Receives all requests of inclusions and deletion from Drug Formulary and all requests of non-formulary additions and discusses them with P&TC members for final decision.
  5. Detects, evaluates and reports to internal and external authorities (SFDA) all adverse drug reactions and provides quarterly reports to P&TC.
  6. Oversees investigation of Medication errors in collaboration with QRM and reviews summary reports before submission to QPSC.
  7. Actively participates in Fakeeh care antimicrobial stewardship program by reviewing compliance with indications of controlled antimicrobials and by ensuring ID approval is obtained for restricted ones and by escalating identified variations. Also provides quarterly reports to P&TC and to ICC on the use of controlled and restricted antimicrobials of physicians with recommendations of the stewardship program and on level of compliance (such as de-escalation or duration of prophylactic antibiotics, etc...).
  8. Conducts daily review of medication orders of all patients admitted to critical care areas, to cardiology and medical / surgical wards and to select patients admitted to other wards for the following: indication ( including based on microbiology sensitivity for antimicrobials and on guidelines for antimicrobial use) ,dosage ( based on review of CR and LFT) , duration ( including any antibiotic dispensed for more than 14 days or any surgical prophylaxis dispensed more than24 hours) , drug to drug interaction, possibility to move from IV to PO.
  9. Reviews medication home history of random sample of patients on daily basis and submits on monthly basis to CMCO the following indicators: percentage of interviewed patients whose physician stated upon admission that they have “NO patient home medication” and who actually “ Had home medications not mentioned by the admitting physician ” and percentage of interviewed patients whose physician stated upon admission that patients had “ medications ordered outside Fakeeh Care” but there were missing medications in the medication history entered by the admitting physician.
  10. Actively participates in the Parenteral Nutrition Service related activities.
  11. Participates in the departmental performance improvement, educational and other clinical activities.
  12. Actively participate in writing, reviewing and revising departmental policies and procedures, and medical guidelines.
  13. Participates in Fakeeh Care committees and taskforces.
  14. Reviews on monthly basis the compliance with restriction on medications ordering to specific specialties and submit reports of compliance to P&TC.
  15. Enrollment in continuous education program regarding medication management practices , high risk medications, medication errors and others.
  16. Educates patients and their families about health conditions, treatments and preventative measures if applicable.
  17. Performs other duties as assigned within the scope of responsibility and requirements of the job.

Job Requirements

Skills and Abilities:

  • Competent computer skills with software applications to include Microsoft Word, Excel.
  • Actively strive to upgrade professional skills through engaging in appropriate professional training and experience.

Experience:

Must have at least one year of inpatient hospital pharmacy experience or completion of residency program.

Education:

Bachelor Pharm D. Degrees, M.S Clinical Pharmacist.

Language:

Good command of oral and written English /Arabic preferable.

Licenses / Certifications:

License for practice as per the regional health regulatory authority e.g. (SCFHS / DHA).
