We’re looking for a Certified Nurse Practitioner (CNP) to join our team and play a pivotal role in providing high-quality healthcare services to our residents. This is your chance to make a real difference in a welcoming and inclusive community that values your expertise and passion.

Your Key Responsibilities:

  • Conduct thorough health assessments, including physical exams, medical histories, and diagnostic testing, to develop personalized care plans.
  • Manage acute and chronic medical conditions, prescribing medications and treatments, and monitoring residents’ responses.
  • Educate and counsel residents and their families on health maintenance, disease prevention, and self-care strategies.
  • Collaborate with an interdisciplinary team—including physicians, nurses, and therapists—to address complex medical needs and ensure optimal resident outcomes.
  • Maintain accurate and timely documentation of assessments, treatment plans, and progress notes, adhering to best practices and regulatory standards.
  • Participate in quality improvement initiatives and continuing education to advance healthcare delivery and enhance your clinical knowledge.

What You Bring:

  • Master’s or Doctoral degree in Nursing with Nurse Practitioner specialization from an accredited program.
  • Current national certification as a Nurse Practitioner (adult-gerontology, family, or related area).
  • Active state licensure and DEA registration, with the ability to practice independently and prescribe medications.
  • Experience in primary care, geriatrics, or long-term care is a plus, but new graduates with relevant clinical rotations are encouraged to apply.
  • Strong clinical assessment and diagnostic skills, particularly in managing diverse medical conditions in older adult populations.
  • Exceptional communication, interpersonal, and teamwork abilities, paired with a compassionate, resident-centered approach.
