About this role

As a Care Partner, you will be the vital link between our clients and the Support at Home program—ensuring they receive the right care, at the right time, in a manner that suits them. This is a dynamic, hands-on leadership role guiding a team of passionate Home Care Workers to deliver high-quality, person-centered care that upholds dignity, independence, and quality of life. You will work closely with clients, their families, and external providers to tailor support and foster strong relationships.

Key responsibilities

  • Assess Client Needs: Conduct assessments to understand physical, emotional, and social needs, and develop tailored care plans.
  • Care Planning: Develop and review person-centered care plans aligned with client goals and funding.
  • Coordinate Services: Organize in-home supports and liaise with external providers.
  • Advocate and Educate: Support clients in understanding their funding, rights, and options, advocating for access to services.
  • Budget Oversight: Monitor budgets and guide clients in effective fund utilization.
  • Relationship Building: Build strong relationships with clients, carers, and providers.
  • Compliance and Safety: Maintain documentation, uphold aged care standards, and manage risks.

What you can bring to the team

You are a natural leader with a passion for community care, confident navigating aged care standards, and thriving in a mobile work environment.

To be successful, you’ll need:

  • Cert III in Individual Support (Ageing) or equivalent
  • Experience leading a team in aged or community care
  • Knowledge of aged care standards, funding, and compliance
  • Current First Aid and CPR certification
  • A reliable, insured vehicle and valid driver’s license
  • Strong communication, organizational, and problem-solving skills
  • Experience in home care or aged care sector

Possess or be willing to obtain:

  • A current National Police Certificate
  • Up-to-date First Aid and CPR certifications
  • National Personal Insolvency Check
