Position Title: Community Nurse Unit Manager

Employment Type: Ongoing full-time

Award: Nurses and Midwives (Victorian Public Health Sector) (Single Interest Employers) Enterprise Agreement 2024 -2028

The Community Nurse Unit Manager (CNUM) is a Senior Leader position that sits within the Continuing Care Directorate working closely with the Community Manager group and other key Senior leaders to ensure optimal management of resources to facilitate high quality person-centred care.

The CNUM is responsible for the management of the following suite of services and roles including;

  • District nursing and community palliative care
  • HARP complex care

This leadership position has a strong focus on delivering and developing services for clients across acute, post-acute and ongoing care, in home and community care. The care is provided by nursing and multi-disciplinary teams with the aim to minimise risk of hospital admission, support recovery of function and independence, and promote self-management of chronic conditions.

In line with value-based health care, the Manager strives to deliver efficient and effective services that improve the health outcomes that matter to clients, improve the consumer experience of receiving care, and improve the staff experience of providing care.

About you

  • Bachelor of Nursing with current AHPRA registration
  • Minimum of 10 years' experience working in a relevant professional area
  • Excellent organisational skills with a proven ability to manage multiple tasks and work to deadlines
  • Current valid driver's licence

Relevant post graduate experience in leadership and management will be highly regarded.
