Evergreen Nephrology partners with nephrologists to transform kidney care through a value-based, person-centered approach. We focus on improving patient outcomes and quality of life by delaying disease progression, shifting care to the home, and accelerating kidney transplants. We enable nephrologists to focus on the right patients at the right time across the full care spectrum with interdisciplinary clinical resources, analytical insights, and services for patients.

Your Role

As a Senior Director, Post-Acute Care and Kidney Care Excellence, you will lead, influence, and ensure proactive clinical care and high-quality outcomes for Evergreen Nephrology patients discharged from facilities to post-acute care settings. You will oversee the strategy, management, and performance of Evergreen’s Post-Acute Care program, ensuring high-quality, coordinated, and continuous care for patients transitioning from inpatient facilities to post-acute centers such as SNFs and from SNFs to home. You will drive performance by creating pathways for optimal discharge dispositions, reducing unnecessary utilization, and monitoring KPIs to measure success and identify improvement opportunities. You will engage internal and external stakeholders to align care management incentives, processes, and execution, aiming to improve clinical quality, outcomes, cost-effectiveness, care delivery consistency, and model fidelity across the interdisciplinary team.

You will foster a teamwork-driven culture of clinical excellence to achieve top performance of clinicians and interdisciplinary teams. This role is fully remote with a requirement to work 9am–5pm in the Eastern time zone.

Role Responsibilities
  • Oversee the delivery of the Post Acute Care (PAC) Program to ensure seamless patient transitions from inpatient facilities to SNFs and from SNFs to home, improving patient outcomes and quality of life
  • Design and deliver the post-acute care delivery strategy for all attributed Evergreen patients based on evidence-based practices and guidelines
  • Establish and monitor Key Performance Indicators (KPIs) to measure PAC initiative effectiveness
  • Be accountable for outcomes that reduce cost of care and improve clinical outcomes
  • Use data and analytics to identify trends, gaps, and opportunities for improvement
  • Create systems and processes to monitor appropriate levels of care and care planning in the SNF setting
  • Identify and allocate essential resources efficiently
  • Drive optimal performance of the PAC clinical team
    • Direct the PAC clinical team’s engagement with SNFs
    • Proactively monitor and manage team performance with real-time feedback
    • Coach and develop team members
    • Inspire and influence the team to deliver best-in-class care, close clinical gaps, and escalate barriers quickly
  • Drive patient engagement in their own care
    • Develop strategies to engage patients in their care
    • Promote patient education and self-management to improve outcomes and reduce utilization
    • Implement patient feedback mechanisms and a process to address feedback
  • Collaborate with key internal and external stakeholders to ensure aligned, high-quality PAC care delivery
    • Develop strong partnership with SNFs to optimize collaboration and align clinical pathways
    • Partner with CMO, central clinical teams, learning and development, and operational leadership to roll out clinical education and training
    • Collaborate with technology and population health operations to implement clinical pathways, workflows, and processes
  • Stay up-to-date on regulatory changes and industry trends
    • Ensure compliance with CMS and payer rules
    • Incorporate industry trends and best practices to enhance care delivery
  • Other duties as assigned
Qualifications
  • You hold an unrestricted RN, NP, or PA state license
  • 10+ years of clinical leadership experience
  • A Master’s degree or higher related to education, quality, public health, business administration, health services research, or related field is preferred
  • Experience in value-based care models and their impact on patient outcomes and costs
  • Proficiency in establishing and monitoring KPIs for PAC initiatives
  • Proven track record of leading improvements in clinical quality and outcomes
  • Experience leading collaborative, consensus-building committees or work groups
  • Strong understanding of clinical standards of care and quality-of-life focus
  • Tenacity to drive initiatives forward and overcome obstacles
  • Dependable and reliable with attendance and availability expectations
  • MS Office proficiency (Outlook, Teams)
  • Ability to work in a primarily remote environment with a dedicated, interruption-free space for PHI/HIPAA
