Job summary

PACS in Poland seeks a Certified Nursing Assistant to provide routine daily nursing care to residents in accordance with assessment and care plans. You will greet residents, assist with daily living activities, and ensure safety and comfort throughout your shift.

Responsibilities include documenting entries, reporting changes to supervisors, and maintaining resident confidentiality while following established policies. CPR preferred; 10th grade education and CNA license required by law.

Qualifications

• Must possess, as a minimum, an 10th grade education.
• Must be a licensed Certified Nursing Assistant in accordance with laws of the state.
• CPR Preferred.
• Ability to read, analyze, and interpret general information and procedures.
• Ability to write reports and communicate with supervisors and residents.

Responsibilities

• Identify residents before administering treatments or meals using wristband or photo card.
• Record entries accurately on flow sheets, notes, and charts.
• Report changes in resident condition to Nurse Supervisor/Charge Nurse promptly.
• Maintain resident confidentiality and report any unauthorized disclosures.
• Assist with daily living activities and personal care per care plan.
• Ensure rooms are prepared for residents and greet/escort them to rooms.
• Assist with transfers, mobility, and post-meal care as instructed.
• Participate in nursing report and follow policies and procedures.
