Job summary

North Bay Regional Center is seeking a Social Services Case Manager to oversee service delivery for clients, including those with intensive needs, ensuring that Individual Program Plans are met and resources are used judiciously.

You will work with an interdisciplinary team to reassess client status, coordinate public and private resources, and monitor service vendors to ensure quality care within a hybrid in‑office/remote framework.

Qualifications

• Bachelor’s Degree in related field (Social Work, Psychology, Counseling, Special Education or Nursing).
• Master’s Degree preferred.
• At least 3 years’ experience with individuals with developmental disabilities.
• Bilingual language ability (Spanish to English) preferred.

Responsibilities

• Manage delivery of services to NBRC clients, including those with intensive needs.
• Ensure Individual Program Plan objectives are met and resources are used effectively.
• Monitor vendor services and verify adherence to service authorizations.
• Conduct annual reassessments with the interdisciplinary team and update plans.
