Job summary

Join MEDVA is seeking a remote RN Case Manager with a California RN license to support clinical teams from the Philippines. You will coordinate care across home health, hospice, and palliative care while maintaining HIPAA-compliant documentation and clear communication with physicians, patients, and families.

This full-time, remote position requires strong clinical judgment, 2+ years nursing and 1+ year case management experience, plus proficiency with EMR systems.

Qualifications

• Active, unrestricted USRN California license.
• At least 2 years of clinical nursing experience.
• At least 1 year of case management or care coordination experience.
• Strong clinical judgment and ability to manage a full caseload independently.
• Excellent written and spoken English, comfortable with physicians and families.
• Experience working in an EMR.

Responsibilities

• Review referrals and intake docs for completeness and eligibility.
• Coordinate care among field nurses, therapists, aides, social workers, physicians, and families.
• Maintain and update patient plans of care; track certification timelines.
• Review field notes and OASIS documentation for accuracy.
• Coordinate with pharmacies and community resources.
• Support discharges, transfers, and post-hospital follow-ups to prevent readmissions.
