Job summary

Limitlessli LLC is seeking a highly organized Quality Nurse Analyst for a remote role. You will review clinical incidents, determine root causes, and communicate with care teams to ensure patient safety and high quality care.

RN with NYS license and a minimum of 2 years nursing experience, plus managed care exposure, are invited. Hours are Mon-Fri 9 AM–6 PM EST, from home, with necessary equipment and stable internet.

Responsibilities

• Review clinical incidents and follow SOPs to determine root cause, risk, and resolution.
• Process interim medical orders and notify physician of patient needs and changes.
• Perform follow-up calls for high-risk recently discharged patients.
• Communicate with client, caregiver, coordination teams, payers, field RNs and stakeholders for patient safety and continuity of care.
• Follow regulatory guidelines to manage incidents to deadlines.
• Share trends and insights from aggregate data for quality improvement.
• Collaborate with assistant director of nursing services to review clinical cases.

We are seeking a highly organized and detail-oriented Quality Nurse Analyst.

This position is remote and offers you the flexibility of working from home.

Essential Role and Responsibilities:

• Review clinical incidents.
• Follow standard operating procedures (SOP) to determine root cause, risk, and resolution for incident.
• Process interim medical orders as needed to notify physician of patient needs and changes in condition such as fall, wound, injury and post-hospitalization
• Perform follow-up calls for patients recently discharged from acute hospitalization and who are considered high risk for readmission
• Communicate as needed to client, caregiver, coordination teams, payers (case management / service coordination), field RNs, providers and key stakeholders to ensure client safety and continuity of care.
• Follow regulatory guidelines to manage clinical incidents to key deadlines.
• Share trends and insights noted in aggregate data to support continuous quality improvement.
• Collaborate regularly with assistant director of nursing services to review clinical cases and client needs.
