Santa Cruz Post Acute is seeking a compassionate and motivated Physical Therapy Assistant to join our rehabilitation team. We offer full-time, part-time, and PRN opportunities with flexible scheduling in a skilled nursing facility focused on resident-centered care.

The PTA will deliver direct treatments per plans of care, assist with exercises and mobility activities, and maintain accurate documentation. California PTA licensure and graduation from an accredited PTA program are required.

Qualifications

• Excellent communication, teamwork, and documentation skills.
• SNF experience preferred, but new graduates are encouraged to apply.
• Graduate of an accredited PTA program required.
• Current PTA license in California required.

Responsibilities

• Deliver direct physical therapy treatments following individualized plans of care by the Physical Therapist.
• Assist residents with therapeutic exercises, gait training, mobility programs, and functional activities.
• Document daily treatment sessions, progress, and responses to therapy in accordance with standards.
• Collaborate with the PT, therapy team, nursing staff, and interdiscliplinary team.
• Educate residents and caregivers on exercise programs, safety strategies, and mobility techniques.
• Maintain compliance with state and federal regulations and professional standards.

Position Overview

Santa Cruz Post Acute is seeking a compassionate and motivated Physical Therapy Assistant (PTA) to join our exceptional rehabilitation team. Our skilled nursing facility is dedicated to delivering high-quality, resident-centered care in a supportive and collaborative environment. We offer full-time, part-time, and PRN opportunities with flexible scheduling.

Key Responsibilities

• Deliver direct physical therapy treatments following individualized plans of care developed by the Physical Therapist
• Assist residents with therapeutic exercises, gait training, mobility programs, and functional activities
• Accurately document daily treatment sessions, resident progress, and responses to therapy in compliance with facility and regulatory standards
• Collaborate with the Physical Therapist, therapy team, nursing staff, and interdisciplinary team members to support resident-centered goals
• Educate residents and caregivers on exercise programs, safety strategies, and mobility techniques
• Maintain compliance with state and federal regulations and professional standards of practice
