Job Overview

We are looking for a compassionate and detail-oriented Patient Services Coordinator to support patients and caregivers throughout their care journey. In this role, you will serve as the first point of contact, guiding patients through the intake process, ensuring accurate scheduling, and maintaining up-to-date records in the CRM. You will also support caregivers with compliance requirements, assist with application documentation, and help streamline communication between patients, staff, and the care team. This position requires excellent communication, strong organizational skills, and the ability to manage multiple priorities while delivering a supportive and professional patient experience.

Responsibilities
  • Serve as the first point of contact, handling inbound inquiries with empathy and professionalism.
  • Pre-qualify patients, explain services, and schedule consultations with healthcare staff.
  • Guide applications through completion, ensuring all documents and requirements are in place.
  • Maintain accurate patient records and scheduling data within CRM systems.
  • Monitor aide clock-ins, follow up on discrepancies, and support scheduling updates.
  • Track caregiver credentials, send reminders for renewals, and assist with compliance requirements.
  • Support light marketing tasks, such as updating referral directories and preparing outreach lists.
  • Collaborate with staff to ensure a smooth intake process and responsive patient support.
Qualifications
  • Previous experience in healthcare administration, patient coordination, or customer service preferred.
  • Excellent communication and interpersonal skills with a compassionate approach.
  • Strong organizational skills and ability to manage multiple priorities.
  • Proficiency with CRM systems, scheduling platforms, and office software.
