Post-Workshop Monitoring (Up to 1 Year)

  • Oversee and consolidate monthly MDT logs and quarterly quality review submissions from participating hospitals.

  • Conduct monthly light check-ins using a standardized pulse survey to assess progress and gather feedback.

  • Facilitate quarterly virtual check-ins to discuss findings, share insights, and address implementation challenges.

  • Provide ongoing support in the use of the toolkit, offering clarification and technical assistance as needed.

  • Assess hospital performance using key performance indicators (KPIs) and recommend those eligible for recognition or additional support.

Reporting & Recognition

  • Consolidate anonymized quarterly progress reports highlighting trends, challenges, and outcomes.

  • Recommend high-performing hospitals for digital certification and Health System Partner (HSP) recognition.

Patient Access Program Coordination

  • Assess, approve, and enroll patients based on established eligibility criteria for the Patient Access Program (PAP).

  • Ensure accurate documentation and coordination with partner hospitals for smooth PAP implementation.
