Job summary

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.

Responsibilities

• Develops, implements, and revises the plan of care in collaboration with physicians and health care professionals.
• Supervises care provided by home health aides and LPNs/LVNs; assigns tasks per regulations.
• Performs supervisory visits and ensures compliance with infection control standards.
• Completes comprehensive assessments (OASIS) and medication reconciliation.
• Documents visits per policy; coordinates with care team and families.
