Makati Life Nurture Medical Center Inc. is seeking a licensed pharmacist to join our clinical team in Metro Manila.

You will review medication orders for safety and efficacy, counsel patients, and support patient care plans in a hospital setting. The role emphasizes collaboration with physicians and nurses, maintaining accurate documentation, staying current with pharmacotherapy guidelines and regulatory requirements, and participating in clinical rounds.

Qualifications

• Licensed Pharmacist
• With a degree in Clinical Pharmacy or Doctor of Pharmacy
• Has a hospital experience but not required
• Is expected to demonstrate the ability to quickly acquire knowledge of departmental and hospital program structure (Mission, Vision and Goal statements) and department policies and procedures.
• Have the ability to demonstrate effective time management skills while working independently with minimal supervision and supervising others.
• Flexibility and adaptability, communication, organization, computer literate, and follow-through skills.

Responsibilities

• Review and evaluate patient medication orders for appropriateness, effectiveness, and safety.
• Collaborate with physicians, nurses, and other healthcare professionals to develop and implement individualized patient care plans.
• Monitor patient outcomes and medication therapy, adjusting treatments as necessary.
• Conduct medication therapy management (MTM) and counseling for patients regarding their medications.
• Educate healthcare staff and patients about drug interactions, side effects, and proper medication use.
• Participate in clinical rounds and case discussions to provide pharmaceutical expertise.
• Conduct research and participate in clinical trials related to pharmacotherapy.
• Stay updated on current pharmaceutical developments, treatment guidelines, and regulations.
• Maintain accurate documentation of patient interactions and medication therapy.
