Aegis Living Granada Hills is seeking a Wellness Nurse to provide quality care to residents with consistent hours outside acute care. The Wellness Nurse supports the nursing team, promotes optimal living, and collaborates with families and physicians on care plans.

The role emphasizes nursing services, staff training, care coordination, and accurate health records to ensure exceptional senior care. Participation in daily rounds and wound/medication management will be expected.

Formación

• Active RN license in good standing.
• Passion for working with seniors.
• Excellent verbal and written communication.
• Ability to work hard, support your team and bring energy.
• Willingness to take a 2-step TB test.

Responsabilidades

• Provide nursing services to residents.
• Participate in training of care staff.
• Coordinate resident care with residents, families, and physicians.
• Communicate care plan changes to staff.
• Communication with families and physicians.
• Documentation and investigation reporting.
• Conduct fall risk, skin, pain, and Fast Scale assessments.
• Wound care.
• Audit eMARS and medication cart.
• Maintain accurate resident health records.
• Medication management.

Responsibilities

What You Will Do:

• Provide nursing services to residents
• Participate in training of care staff
• Coordinate resident care with residents, families, and physicians

Qualifications
