A healthcare provider is seeking a Hospital at Home Nurse Practitioner to deliver secondary level care within patients' homes. You will work in a team, providing clinical support and liaising with various agencies. This role is vital for ensuring high-quality care and requires a commitment to working legally in the UK. The position offers both full and part-time hours.

The Hospital at Home service provides secondary level care within the patient’s own home or care setting. The Nurse Practitioner (NP) will work as part of the Hospital at Home team providing clinical care to patients who would otherwise be in a hospital setting. This post is integral to ensuring safe, effective, high quality care delivery in the patient’s home environment.
As a NP you will liaise with several professionals/agencies, both internal and external daily including GP practices, local authority teams and the voluntary sector. Along with helping towards continually developing a sustainable and appropriately skilled workforce with a reduction in dependency on conventional medical models of care provision.
Currently the service is 7 days, 0800-1800 daily. As a NP within H@H your post would work Monday – Friday with weekend working.
Salary information: Please note: the salary quoted is for full-time hours (37 hrs per week). For appointments to part-time hours, the salary will be pro-rata.
