Job summary

Health New Zealand - Te Whatu Ora Tairāwhiti Gisborne Hospital is seeking a Gateway Programme Co-ordinator on a part-time basis (0.5FTE). You will coordinate health assessments and follow-ups for tamariki referred to Gateway, ensuring interagency service agreements are completed and integrated care is delivered.

You will work within the Women, Child & Youth Service to improve health outcomes for children and young people in Gisborne and the wider Tairāwhiti region, promoting collaboration,

Qualifications

• NZ Registered Practitioner with current experience working in child and/or youth health.
• Awareness and understanding of the Treaty of Waitangi and its application in the health setting.
• Proven community partnership approach/engagement in Tairāwhiti or other rohe.

Responsibilities

• Coordinate health assessments for Gateway referrals from Oranga Tamariki.
• Coordinate follow-up required and ensure Interagency Services Agreements are completed.
• Lead new ways of working and support a collaborative, equitable approach to care for tamariki and whānau.
