Job summary

Pinnacle Midlands Health Network is seeking a Community Care Coordinator to champion Māori health equity across the Tairāwhiti region. You will work across Pinnacle Midlands Health Network, Te Rūnanga o Tūranganui a Kiwa, and Ngāti Porou Oranga, coordinating wrap-around care for whānau and communities.

You will coordinate across health and social services, ensure continuity of care, and act as the voice for primary health within the partnership.

Qualifications

• Registration as a health professional with a current annual practising certificate.
• Primary or community healthcare experience.
• A strong understanding of health inequities in Aotearoa New Zealand.
• Excellent communication, time management, and organisational skills.
• Commitment to Tikanga Māori and Te Tiriti o Waitangi.

Responsibilities

• Champion Māori health and equity, prioritising whānau with the greatest need.
• Coordinate care across health and social services for individuals, whānau, and communities.
• Facilitate smooth transitions and continuity of care across providers.
• Identify and support those at greatest risk of poor health outcomes.
• Act as the voice of primary health within the partnership, supporting Iwi aspirations.
