Pinnacle Midlands Health Network in the Tairāwhiti region seeks a Community Care Coordinator who champions Māori health equity and improves outcomes for whānau. The role connects across health and social services to coordinate wrap-around care and build strong partnerships across agencies.

You will identify those at greatest need, support transitions of care, and be the voice of primary health within the partnership, while upholding Tikanga Māori and Te Tiriti o Waitangi.

Qualifikationen

• Registration as a health professional with current APC.
• Primary or community healthcare experience.
• Strong understanding of health inequities in Aotearoa NZ.
• Excellent communication, time management and organisation skills.
• Commitment to Tikanga Māori and Te Tiriti o Waitangi.

Aufgaben

• Champion Māori health equity and coordinate care across health and social services.
• Coordinate wrap-around care for individuals, whānau and communities.
• Facilitate smooth transitions and continuity of care across providers.
• Identify and support those at greatest risk of poor outcomes.
