Job summary

Dorset HealthCare University NHS Foundation Trust is seeking a Physical Health Support Worker to improve physical health for people living with serious mental illness within primary and secondary care, helping them access appropriate levels of care.

You will receive high‑quality training in mental health awareness, physical health promotion and behaviour change, with protected time for training, supervision and multidisciplinary team work.

• Experience in physical health assessments (clinic and home settings)
• Ability to signpost services to improve physical health
• Ability to set goals with individuals to improve physical health
• Willingness to work across primary and secondary care teams

Responsibilities

• Provide comprehensive physical health assessments (clinic and home settings)
• Signpost individuals to services to improve their physical health
• Set goals with individuals to improve their physical health
• Develop and promote physical health care goals and plans to support self-management
• Promote relationships with partner organisations and communities to support sustainability
• Conduct annual physical health monitoring and reviews for people with serious mental illness
