We have an exciting opportunity for an experienced Senior Social Worker to join our Social Work team in a permanent, full-time (1.0 FTE) position. This role offers the opportunity to further develop your leadership capabilities while working across the Te Toka Tumai region with adults living with chronic and long-term health conditions. Combining advanced clinical practice with leadership development, this role provides opportunities to contribute to service development and quality improvement initiatives while supporting the delivery of high-quality, person-centred care in the community.
The Community Allied Health Team is a large multidisciplinary service for adult clients who have physical health and disability needs and live in the central Auckland region. Our Social Workers work in multidisciplinary locality teams and are committed to the holistic delivery of health services within the Te Toka Tumai|Auckland community providing integrated care that is coordinated around the client and their needs. The role may be based at either Greenlane Clinical Centre or Point Chevalier Clinical Centre, depending on service requirements.
