Job summary

Health New Zealand | Te Whatu Ora is seeking Senior Coordinators to lead local district teams delivering intensive community wraparound services for tangata whaiora exiting prison with high risk of disengagement or reoffending. Roles are based in Hamilton, Rotorua, Tauranga, Gisborne and New Plymouth, reporting to a central Lead Coordinator.

The Senior Coordinator will oversee caseloads, gate support coordination, MYPLAN completion, and coordination with ACMHT, Probation, MSD and Iwi/NGO

Qualifications

• Minimum 5 years post graduate experience in mental health, community health or primary care.
• Demonstrated experience delegating to and supporting clinical and non-clinical staff.
• Whanau centred, collaborative and partnership-based approach.
• Competency with te ao Maori, tikanga, and te reo Maori or willingness to learn.

Responsibilities

• Lead district team of Te Manawa Taki Intensive Community Wraparound.
• Manage caseload, pre-release planning, gate support coordination and post-release reintegration oversight.
• Direct work to the district Clinician and CSW; ensure MYPLAN completion and caseload quality.
• Maintain relationships with ACMHT, Probation, MSD, Kāinga Ora, Iwi NGO providers.
