A leading healthcare organization in Southland is seeking a part-time Case Manager to oversee health and development checks for whānau. You will provide vital community health support and work alongside Registered Nurses and health workers. This role requires a confident and flexible practitioner with a focus on building relationships and addressing complex social needs. Whānau Āwhina Plunket is the largest service in Aotearoa supporting the health and wellbeing of tamariki under-five and their whānau. We provide clinical support through traditional well child checks, 24/7 telehealth via PlunketLine, and community support such as playgroups where whānau can gather and form lasting positive relationships within their community.
You will act as a case manager for between 650 – 700 whānau across the Invercargill area, conducting health and development checks in homes and at our Kelvin St clinic. For whānau who need additional support, you’ll work in partnership with our team of Registered Nurses, health workers and other internal and external services to ensure each pēpi has what they need to thrive.
We offer a part-time arrangement (4 days a week or 5 shorter days) on a fixed term contract until May 2026. Full-time annual salary for our Registered Nurses ranges from $66,222 ($33.51 per hour) to $94,842 ($48 per hour), depending on experience.
Once you pick up your car and head out for the day, this role offers a fair amount of autonomy. You need to be a confident practitioner who is flexible enough to manage the dynamic environment of community work. You’ll have a warm and non-judgemental approach which will enable you to connect with and understand the needs of each whānau. In a cost of living crisis, social needs often co-exist with clinical advice; therefore, screening for and supporting with complexities such as family violence, substance use and mental health with sensitivity is a constant part of our role.
