With a strong focus on family (whānau) support, they are dedicated to ensuring patients receive the highest standard of care, comfort, and dignity during this sensitive stage of life.

Responsibilities
  • Manage and coordinate palliative care for patients in the community
  • Collaborate with the Clinical Services Manager to deliver high-quality care
  • Conduct daily check-ins with patients and provide home visits as needed
  • Support patients with complex care needs, ensuring comfort and dignity
  • Liaise with GPs, Allied Health, carers, and whānau to ensure holistic, patient-centered care.
Qualifications
  • Current New Zealand Nursing Annual Practising Certificate (APC)
  • Experience in community and/or palliative care settings
  • Genuine passion for palliative care, with a holistic approach to patient and whānau support
  • Compassionate, respectful, and ethical in all interactions
  • Strong communication and patient assessment skills
  • Able to work independently while contributing to a collaborative team environment
  • NZ residency or valid NZ work visa required
