Job summary

Health New Zealand | Te Whatu Ora in Nelson is seeking an experienced Addictions Case Manager (allied health or registered nurse) to provide assessment, care coordination, and recovery-focused interventions for people with substance use and co-existing mental health concerns in community settings.

You will work with clients, whānau, and a wide network of health and social services, offering withdrawal support when appropriate and contributing to a holistic, person-centred recovery journey.

Qualifications

• Current NZ Annual Practising Certificate in your professional discipline.
• Experience in addictions or mental health settings is preferred.
• Ability to work within a recovery-oriented framework with clients and whānau.

Responsibilities

• Conduct comprehensive assessments and care coordination.
• Develop treatment plans and recovery-focused interventions.
