Job summary

Health New Zealand | Te Whatu Ora in Nelson is seeking an experienced Addictions Case Manager (allied health or registered nurse) to provide assessment, care coordination, and recovery-focused interventions for people with substance use and co-existing mental health concerns in community settings.

You will work with clients, whānau, and a wide network of health and social services, offering withdrawal support when appropriate and contributing to a holistic, person-centred recovery journey.

Qualifications

• Current NZ Annual Practising Certificate in your professional discipline.
• Experience in addictions or mental health settings is preferred.
• Ability to work within a recovery-oriented framework with clients and whānau.

About the Role

We are seeking an enthusiastic andexperienced Addictions Case Manager, who is either an allied healthprofessional, or registered nurses, with experience, preferably within Addictionsor mental health setting. As a Case Manager, you will workcollaboratively with clients, their whānau, and support networks to providecomprehensive assessment, care coordination, treatment planning, andrecovery-focused interventions. You will work alongside individualsexperiencing substance use challenges, often in combination with mental healthconcerns, supporting them through withdrawal and recovery. This includesfacilitating medicated withdrawal where appropriate, promoting freedom fromaddiction, enhancing wellbeing, and empowering people to live fulfilling lives.The role offers variety andautonomy, with opportunities to work across community settings and alongside awide range of health and social service providers.

• Excellent organisational skills, sound clinical judgement, and the ability to manage a varied caseload.
