Job summary

ProCare Fresh Minds in Waitakere, Auckland, NZ seeks a Health Improvement Practitioner to empower individuals and whānau through brief interventions within a general practice setting. You will join a supportive, multidisciplinary team delivering evidence-based therapies in two fast-paced environments.

Role offers permanent part-time opportunities (0.6–1.0 FTE). Training and mentoring are provided within the Te Tumu Waiora programme, with a focus on equity and community wellbeing.

Qualifications

• Must be a fully registered mental health professional (not provisional).
• Training in talking therapies such as ACT, FACT, or CBT.
• Able to work in two fast-paced GP environments and adapt to changing needs.
• Excellent typing/IT skills and ability to enter clinical notes into PMS systems.
• Respect for diverse cultures and communities.

Responsibilities

• Provide brief psychological interventions, particularly ACT/FACT, and group programmes.
• Offer consultation, education, and advice to General Practice teams; identify unmet patient needs.
• Develop intervention pathways with GPs and nurses based on clinic needs.
