Tamaki Health's Wellness Support Team in Auckland is seeking a Health Improvement Practitioner to join our award-winning team. In this primary care role, you will work alongside GPs, nurses and Health Coaches to deliver brief, evidence-based mental health and wellbeing support.
Using the FACT model, you will be embedded in the clinic team, providing coordinated care, warm handovers, and timely interventions while promoting culturally safe practice and equitable outcomes for diverse communities.

The Wellness Support Team is seeking a Health Improvement Practitioner to join our award-winning team.
In this innovative primary care role, you will work alongside GPs, nurses, and Health Coaches to deliver brief, evidence-based mental health and wellbeing support, while growing your clinical skills and making a meaningful difference for patients and whānau every day.
As a Health Improvement Practitioner, you will work within the nationally recognised Health Improvement Practitioner (HIP) model, using Focused Acceptance and Commitment Therapy (FACT) to support people experiencing a broad range of mental, physical, behavioural, and social wellbeing concerns. Embedded within the clinic team, you will play a key role in delivering integrated care through warm handovers, timely interventions, and strong multidisciplinary collaboration.

About the Wellness Support Team
Established in 2012, the Wellness Support Team provides integrated mental health and wellbeing support within primary care settings across Auckland. Our team includes Health Improvement Practitioners, Health Coaches, support workers, and clinical leaders who share a commitment to accessible, person-centred care. Through a stepped-care approach, we help ensure patients and whānau receive the right support at the right time.
The programme has been recognised nationally, receiving the award for Best Mental Health Programme at the New Zealand Primary Healthcare Awards.
