Job summary

Frontpoint Health Home Health OT position is a PRN per visit role in Texas. The Occupational Therapist will provide direct patient care to homebound clients, develop and adjust treatment plans, monitor progress, and help patients achieve greater independence and improved quality of life.

Collaborate with a care team, participate in conferences, supervise aides on-site, and maintain thorough progress notes. Requires Texas OT license, CPR/BLS certification, and a valid driver’s license; knowledge

Qualifications

• Current Texas OT License.
• Current Certification in BSL/CPR.
• Valid Texas Driver's License and Automobile Insurance.
• Knowledge of Home Care Home Base software is a plus but not required.

Responsibilities

• Develops and revises plan of care in consultation with physician and other care-team members.
• Provides therapeutic treatments and evaluation of equipment needs as applicable.
• Participates in clinical conferences and in-service programs.
• Conducts on-site supervisory visits of certified occupational therapy assistants.
• Advises and consults with the family and other agency personnel as applicable.
• Prepares clinical and progress notes.
• Protects and honors patient and coworker confidentiality and right to privacy.
• Evaluates outcomes of care.
• Plans for discharge.

Home Health - PRN - Per Visit - Occupational Therapist

Location: Cleburne, TX 76031

The Home Health - Occupational Therapist will be responsible for providing direct patient care to home bound patients. The Therapist will work with patients to develop treatment plans, monitor progress, and adjust treatment as needed and helping them achieve greater independence and improve their quality of life. The Occupational Therapist will also work closely with other healthcare professionals to ensure that patients receive comprehensive care.
